When you become a patient of Boulder Kind Care, we offer registration assistance at no extra fee.
How do I become a patient?
A doctor must write you a marijuana recommendation. Some patients are reluctant to ask their primary physician for a referral. We can refer you to a physician to perform an independent, confidential evaluation.
Send the doctor’s recommendation to the Colorado Department of Public Health and Environment, along with your application and $90 fee. When you designate Boulder Kind Care as your caregiver we will help you with all the necessary forms and make copies for your records.
Once approved, you will receive a state-issued medical marijuana card and be included in the medical marijuana registry. Your ID card is valid for one year, after which you’ll need to apply again.
